Імунні механізми виникнення та клінічні особливості перебігу неінфекційного увеїту

Анотація

Увеиты занимают от 5 до 13 % в структуре слепоты и слабовидения, они могут быть самостоятельным идиопатическим заболеванием, или же составной частью системных ревматических заболеваний как в детском, так и во взрослом возрасте. Научное общество SUN определило критерии активности воспалительного процесса, создало распределение форм увеита по типу течения и определило алгоритм лечебной тактики. В основе развития увеитов лежат нарушения иммунных механизмов толерантности и возникновения аутоагрессии к собственным антигенам глаза. Иммунная толерантность к антигенам глаза нарушается при недостаточности регуляторной субпопуляции CD25 + T- лимфоцитов, активации В-лимфоцитов с выработкой специфических антител к структурам увеального тракта. Для лечения увеитов применяют кортикостероиды, нестероидные противовоспалительные препараты, цитостатики и иммунобиологические препараты. Однако не существует клинических рекомендаций, которые бы объединяли перечисленные методы лечения увеита. Анализ корреляционной связи между степенью нарушения иммунной системы и типом течения увеита позволит выработать алгоритм лечения воспалений сосудистой оболочки глаза.
