Diagnosis and Treatment of Acanthamoeba Keratitis

Цель: Описать клиническую картину, диагностику и лечение пациентов с установленным диагнозом Акантамебного кератита (АК).
Материал и методы: проведен ретроспективный анализ 14 пациентов (14 глаз) с диагнозом АК, получавших лечение в Centro Visi?n в период с июля 2008 по июнь 2012 г. Во всех случаях диагноз был подтвержден наличием мазка и / или культуры. Пациенты были разделены на две группы: с ранним и поздним установлением диагноза. Лечение проводилось с применением пропамидина и полигексаметилен бигуанида. По окончании лечения фиксировали конечную остроту зрения и длительность проводимой терапии.
Результаты. Заболевание преобладало в возрастной группе от 21 до 40 лет (9 случаев). Только в двух случаях (14,3%) изначально был выставлен правильный диагноз АК. Одиннадцать пациентов (78,6%) пользовались контактными линзами. Наиболее распространенным признаком данного заболевания было наличие диффузного инфильтрата (62,3%); периневральный инфильтрат наблюдали только в одном случае. Правильный диагноз был поставлен пяти пациентам в течение первых тридцати дней (группа с ранним установлением диагноза) и девяти пациентам после 1 месяца наблюдения (группа с поздним установлением диагноза). Средняя острота зрения в группе с ранней диагностикой АК составила 0,5, а в группе с поздней — 0,05. Средняя продолжительность лечения в первой группе (ранняя диагностика) составила шесть месяцев и во второй группе (поздняя диагностика) десять месяцев.
Выводы: В большинстве случаев (85,7%) сначала был выставлен ошибочный диагноз кератита другой этиологии. Ранняя диагностика АК способствует сохранению зрительных функций и уменьшению сроков необходимой терапии.
